Microsoft Teams Integration

Formspace integrates directly with Microsoft Teams, letting your team build forms, view responses, and embed live forms into channels and tabs — all without leaving the Teams environment.

The Formspace Teams app supports two modes:

Your Microsoft Teams administrator may need to approve the Formspace app before it’s available in your organization. Once approved:

  1. Open Microsoft Teams.
  2. Click the + button at the top of a channel or chat to add a tab.
  3. Search for Formspace in the app catalog.
  4. Click Add to install the app.

When you open the Formspace tab for the first time, you’ll be prompted to sign in. The app supports:

  • Microsoft single sign-on (SSO) — If your Teams account is linked to a Formspace account, you’ll be signed in automatically. This is the recommended approach for the smoothest experience.
  • Email sign-in — If SSO isn’t available, you can sign in with your email and a one-time verification code.

After installing the app and signing in, you’ll see the configuration screen. Here you choose how the Formspace tab should work.

Select Dashboard to embed the full Formspace dashboard. This gives you access to:

  • The overview dashboard with stats and charts.
  • The forms list where you can create, edit, and manage forms.
  • The form builder with all its tabs (Designer, Preview, Theme, Translation, Publish, Submissions).
  • Organization settings and member management.

This is ideal for a team channel where multiple people collaborate on building and managing forms.

Select Embed Form to pin a single published form as a tab. When you choose this mode:

  1. A dropdown appears listing all your published forms.
  2. Select the form you want to embed.
  3. Click Save.

The tab will display that form directly, so anyone who opens the tab can fill it out and submit a response without leaving Teams.

If you belong to multiple Formspaces, use the organization dropdown in the top-right corner of the configuration screen to switch between them. The tab will be configured for whichever Formspace is currently selected.

Once configured in Dashboard mode, the tab works just like the regular Formspace web app. You can:

  • Navigate between Overview, Forms, and Settings using the sidebar.
  • Create new forms and edit existing ones in the full form builder.
  • View and manage responses in the submissions table.
  • Publish forms and configure sharing settings.
  • Manage team members and subscription settings.

The dashboard adapts to the Teams window size, and the sidebar collapses automatically on smaller screens.

When configured in Embed Form mode, the tab displays your selected form. Respondents can:

  • Fill out the form directly within the Teams tab.
  • Navigate multi-page forms with Next and Previous buttons.
  • Upload files if the form includes file upload fields.
  • Submit their response without leaving Teams.

The form respects all your access control settings. For example, if the form requires email verification, respondents will be prompted to verify before submitting.

To change the tab’s mode or select a different form:

  1. Click the dropdown arrow next to the tab name at the top of the channel.
  2. Select Settings.
  3. The configuration screen reappears where you can switch between Dashboard and Embed Form modes, change the selected form, or switch Formspaces.
  4. Click Save to apply your changes.
  • Pin the Dashboard tab in your main team channel so everyone has quick access to form management.
  • Create separate Embed Form tabs for each form you want to distribute within a channel — for example, a weekly standup form or a feedback survey.
  • Use Organization access mode with Microsoft tenant verification on your forms to restrict responses to people in your Teams organization. This pairs perfectly with the Teams integration.
  • Enable email notifications so team members get notified in their inbox when responses come in, complementing the in-Teams experience.

This message appears if you try to access the Teams configuration page outside of Microsoft Teams. The configuration screen is only functional within the Teams app.

If automatic sign-in doesn’t work:

  1. Try clicking Sign in with Microsoft on the login screen.
  2. Make sure your Microsoft account is linked to your Formspace profile (see Managing Your Profile → Linked Accounts).
  3. If you don’t have a Formspace account yet, sign up first and link your Microsoft account, then return to Teams.

The Embed Form option only shows published, non-closed forms. To fix this:

  1. Switch to Dashboard mode.
  2. Create a form and publish it.
  3. Reconfigure the tab and switch to Embed Form mode — your published form should now appear.
